    Rehabilitation Services

  • Physical Therapy

    Physical therapists apply research and proven techniques to help people get back in motion. They are trusted health care professionals with extensive clinical experience who examine, diagnose, and then prevent or treat conditions that limit the body's ability to move and function in daily life. Physical therapists provide care for people in a variety of settings, including hospitals, private practices, outpatient clinics, home health agencies, schools, sports and fitness facilities, work settings, and nursing homes.

    Occupational Therapy

    In its simplest terms, occupational therapists and occupational therapy assistants help people across the lifespan participate in the things they want and need to do through the therapeutic use of everyday activities (occupations). Common occupational therapy interventions include helping people recovering from injury to regain skills, and providing supports for older adults experiencing physical and cognitive changes. Occupational therapy practitioners have a holistic perspective, in which the focus is on adapting the environment to fit the person, and the person is an integral part of the therapy team.

    Speech Language Pathologies

    Working with the full range of human communication, speech-language pathologists (SLPs) evaluate and diagnose speech, language, cognitive-communication, and swallowing disorders and treat such disorders in individuals of all ages, from infants to the elderly. In addition, SLPs develop new methods and equipment to evaluate problems, establish more effective treatments, and investigate behavioral patterns associated with communication disorders. SLPs often work as part of a team, which may include teachers, physicians, audiologists, psychologists, social workers, rehabilitation counselors, and others.

    Skilled Nursing Services

  • Registered Nurses/Licensed Practical Nurses

    Includes skilled nursing care, nursing assessments, care management, IV medication administration, medication prefills, wound care, cardiovascular assessments, public health nursing, hospice care, private duty clinical assessments and nursing care.

    Home Health Aides

    Includes assistance with personal care, activities of daily living, assistance with range of motion exercises, assistance with transfers, medication reminders and assistance with taking medications as ordered by physicians in prefilled containers, and home care management under the supervision of a nurse and following a plan of care that has been established by the nurse. Services include hospice, and Alzheimer’s care.

    Medical Social Services

  • Medical Social Workers play a vital role in the care team. After a patient is admitted to Home Health or Hospice Services, a Medical Social Worker may conduct an assessment to help determine patient needs and identify any limitations, barriers, or safety issues that might impede recovery, educate patients and families regarding community resources and support, and assist with long range planning, including advance care planning and directives, and overall care opportunities.

    Our Medical Social Workers work with the patients and families to keep patients at home in a safe and healthy environment.

    Medical Social Services include:

    • Community resource planning and coordination
    • Advance directives (Power of Attorney for Health Care or Living Will)
    • Assistance in emotional adjustments to life changes
    • Crisis intervention
    • Assistance with applications for financial aid
    • Teaching regarding coping with loss and change

    Nutritional Counseling

  • Nutrition Counseling can help you reach and maintain your peak health, decrease doctor visits and hospitalizations, and reduce prescription drug costs.

    Our nurses will provide information regarding specialty diets and will also refer for dietary consults through your private physician if needed.

    Specialty Services

  • INR Monitoring

    A test called International Normalization Ratio (INR) is the lab test that is routinely performed to monitor warfarin levels. Warfarin monitoring is extremely important to a patient's health.

    CoaguCheck XS System

    The CoaguChek® XS system is a convenient, portable and user-friendly instrument for monitoring oral anticoagulation therapy. It determines the INR value (International Normalized Ratio) from a drop of capillary whole blood –it’s simple, precise and reliable.

    Telemonitoring

  • Telemonitoring is a system that is set up in the patient's home to monitor vital signs on a daily basis. This information is transmitted via computer and monitored by a registered nurses everyday. The process is easy and convent for the patient and allows our nurses to monitor for changes in condition, report changes to the doctor, so that patients can remain at home and avoid unnecessary hospitalizations.


Before Surgery

We show our care at Athena Home & Hospice, our staff will show our care and attention to detail by going above and beyond patients' expectations. We urge patients to call us before surgery to discuss any concerns so we can help reassure and guide them through the pre and post-op process.

After Surgery

The initial home care assessment is conducted within our patients homes with our nurses and physical therapists. We review home care needs such as manual therapy, functional mobility and home exercise. We provide training regarding medication management, diet and monitoring of vital signs. We coordinate other relevant services with our patients physicians, while providing physical and emotional support.
